A Case of Acute D.D.S. Intoxication with Various Complications



Abstract
In a case of acute D.D.S. intoxication, unusual various complication including methemoglobinemia, exfoliative dermatitis, toxic hepatitis, toxic myocardiopathy, and toxic nephropathy were presented.
A 21 year-old Korean lacy was carried to emergency room of this hospital under the comatose state after ingestion of logm. of D. D.S. for suicide. Positive physical signs were cyanosis of all over the body surface, and increased vesicular breathing sound on the entire lung field. Laboratory data disclosed R BC 3.56 Mil. /cu. mm., HE 10.. 5gm. %, Hct. 36 vol. %, WBC 16, 100/cu. mm., reticulocyte 3. 0%, proteinuria ( -I- to . + + +) and microscopic hematuria (many red blood cell/H. P. F. ). Examination of blood chemistry showed N.P.N. 62mg. /100ml., B.U.N. 40mg. /100m1. creatinine 2. 5 mg. / 100ml., S-GOT 68 units, and normal values of K., Na., Cl., and Ca. in the serum.
She was treated with methylen blue, ascorbic acid, and whole blood transfusion, however, died out unfortunately on the fourth hospital day at which time necropsies of the heart, liver, and kidney were performed, and the above unusual various complications were proved histologically.
